Magnesium L-threonate is the only form with clinical evidence for raising brain magnesium levels — it crosses the blood-brain barrier where others don't. Glycinate is the best-absorbed form for systemic supplementation, especially for sleep and muscle relaxation. Different goals, different forms.
Form matters more than dose
Almost half of Americans consume less than the RDA for magnesium.[1] Correcting deficiency has legitimate cognitive, cardiovascular, and sleep benefits. But the form of magnesium you take determines where it goes and how well it's absorbed.
Magnesium oxide (the cheap form in most drugstore supplements) has ~4% elemental magnesium absorption. Most of it acts as an osmotic laxative and produces stool.
Magnesium glycinate (magnesium bound to glycine) has ~80% absorption, is well tolerated at higher doses, and glycine itself has calming, sleep-supporting effects.
Magnesium L-threonate (Magtein — bound to a metabolite of vitamin C) is the only form shown to raise CSF and brain magnesium levels in animal studies and confirmed in one human trial.
The L-threonate story
The foundational L-threonate research came out of Guosong Liu's MIT lab. Slutsky 2010 tested magnesium L-threonate in aged rats — brain magnesium levels increased, synaptic density in prefrontal cortex increased, and both short- and long-term memory improved on multiple behavioral tests.[2]
The Liu human trial (2016) tested 1.5-2 g/day of magnesium L-threonate in 44 older adults with subjective cognitive complaints over 12 weeks. The treatment group showed significant improvements on the Trail Making Test A and B, digit span backwards, and other executive function measures.[3] This is a small trial — but it's the only human data for this form's cognitive effect.
Magtein is the branded L-threonate ingredient — the version used in the published human trial. Generic L-threonate has emerged but has no clinical validation.

Where each fits
Magnesium L-threonate is worth the premium if:
- You're specifically trying to raise brain magnesium (cognitive aging, mild cognitive complaints)
- You're already replete on systemic magnesium and want the brain-specific effect
- Cost isn't the constraint and you want the form with published cognitive trial data
Magnesium glycinate is the better default if:
- You have known or likely magnesium deficiency (low intake, high sweat losses, medications like PPIs or diuretics that deplete magnesium)
- Sleep is a primary goal (the glycine bonus helps)
- Muscle cramps, tension, or restless legs
- You want a well-tolerated, well-absorbed form at reasonable cost
Some cognitive protocols combine both: Magtein L-threonate in the morning (144 mg elemental / 1.5 g Magtein) plus magnesium glycinate at bedtime (200-300 mg elemental).
Total daily elemental magnesium in the 350-450 mg range is generally well tolerated. The RDA is 320 mg for women and 420 mg for men, so this stack aligns with meeting or slightly exceeding baseline needs while also providing the L-threonate brain-specific dose.
Forms to skip
- Magnesium oxide: poorly absorbed (~4%). Most goes through you as stool softener. Cheap but almost worthless for tissue repletion.
- Magnesium citrate: better absorbed than oxide but strongly laxative at higher doses. Fine for occasional constipation, not for daily supplementation.
- Magnesium malate: reasonable absorption but no compelling data supporting the malate part's touted "energy" benefit.
- Topical magnesium (spray, oil, Epsom bath): zero controlled evidence for meaningful systemic absorption despite popular claims.
The takeaway
If you're taking one magnesium supplement and cost matters: glycinate. If your primary goal is brain-specific cognitive support and you're already replete: L-threonate (Magtein specifically). If you're doing a serious cognitive stack: consider both.
Whichever you pick, ignore magnesium oxide — the "cheap magnesium" savings are illusory when 96% of it never makes it into your tissues.
