The energy pathway split
Creatine is stored in tissue as phosphocreatine, which donates a phosphate to ADP to regenerate ATP during high-demand moments. Muscle uses it during heavy lifts. The brain uses it during demanding cognitive tasks — especially under stress, sleep deprivation, or hypoxia.
Citicoline (CDP-choline) raises frontal-lobe high-energy phosphates (ATP and phosphocreatine) through a different route: it supports phospholipid synthesis, which improves mitochondrial membrane efficiency. The Rae 2003 trial is often the citation of choice for creatine cognitive research. 45 young adult vegetarians took 5 g creatine or placebo daily for six weeks in a crossover design. The creatine group showed significant improvement on working memory (backwards digit span) and Raven's Advanced Progressive Matrices.[2] The vegetarian population matters here — dietary creatine intake is near zero without meat consumption, so supplementation shows a bigger cognitive delta.
McMorris 2006 tested 5 g creatine for 7 days in sleep-deprived healthy adults. The creatine group showed significantly reduced cognitive decline during a 24-hour sleep deprivation protocol on tasks requiring sustained attention and complex mental processing.[3]
Silveri 2008's citicoline finding used 4-week supplementation at 500 mg/day. The 31-P MRS measured a 14% increase in phosphocreatine and a 32% increase in beta-nucleoside triphosphate (mostly ATP) in the frontal lobe.[1]
When each one earns its place
Creatine wins acutely under stress. Sleep deprivation, high-altitude, extended cognitive load, endurance sports. It's also the highest-signal supplement for vegetarians and vegans since they can't get it from diet.
Citicoline wins chronically for cognitive aging. The frontal-lobe phosphate metabolism story matters most for people concerned about age-related decline. It also has stronger data in mild cognitive impairment and post-stroke recovery.

A common configuration: 5 g creatine monohydrate daily (in water, anytime) plus 250-500 mg citicoline (Cognizin) with breakfast. Both are cumulative — expect 3-4 weeks before either is fully saturated in tissue.
Skip loading phases for creatine — the 5 g/day maintenance dose gets you fully saturated in about 4 weeks with no GI upset. Loading (20 g/day for 5 days) works faster but causes GI issues in many people.
If you don't eat meat, creatine is arguably the highest-value single supplement you can take — cognitive, athletic, and muscle-preservation benefits, all at ~$10 a month. The Rae 2003 effect size in vegetarians is substantially larger than what meat-eaters see.
Creatine monohydrate is the form used in essentially every trial. Ignore "creatine HCl," "buffered creatine," and other forms — monohydrate has 20+ years of trial data and no formulation has consistently beaten it.
